Abstract

Obesitas telah mencapai proporsi epidemik secara global. Peningkatan prevalensi obesitas dihubungkan dengan perubahan gaya hidup. Populasi Bali telah terpapar perubahan gaya hidup selama tiga dekade terakhir karena perkembangan pariwisata, menyebabkan tingginya prevalensi obesitas. Polimorfisme nukleotida tunggal (SNP) gen fat mass and obesity associated (FTO) rs9939609 dilaporkan memiliki asosiasi dengan obesitas pada banyak populasi, meskipun memiliki prevalensi yang berbeda-beda. Penelitian ini bertujuan untuk menentukan asosiasi varian rs9939609 dengan obesitas pada populasi Bali. Studi dilakukan pada 574 spesimen tersimpan berasal dari empat desa (Legian, Penglipuran, Nusa Ceningan, dan Pedawa). Klasifikasi obesitas berdasarkan indeks massa tubuh (IMT) dan obesitas sentral berdasarkan lingkar pinggang (LP) mengikuti klasifikasi WHO untuk orang Asia: IMT ≥ 25 kg/m2 baik pada pria dan wanita, LP ≥ 90 cm untuk pria dan LP ≥ 80 cm untuk wanita. Genotipe rs9939609 ditentukan menggunakan sistem amplifikasi mutasi refrakter (ARMS). Analisis asosiasi varian rs9939609 dengan obesitas dan obesitas sentral dilakukan dengan uji regresi logistik ordinal dengan penyesuaian usia dan jenis kelamin. Frekuensi alel minor varian rs9939609 sebesar 0.43, dan distribusi genotipe tidak menyimpang dari kesetimbangan HardyWeinberg (p = 0.396). Varian rs9939609 berasosiasi dengan obesitas pada model dominan (koefisien = 0.44, p = 0.040). Tidak terdapat asosiasi antara varian rs9939609 dengan obesitas sentral. Variasi gen FTO rs9939609 berasosiasi dengan obesitas pada populasi Bali. Obesity has reached epidemic proportions globally. Increased prevalence of obesity has been linked to changes in lifestyle. The Balinese has been exposed to lifestyle changes for the last three decades due to tourism development, resulting in high prevalence of obesity. The rs9939609 single nucleotide polymorphism (SNP) of the fat mass and obesity associated (FTO) gene was reported to associate with obesity in many populations, despite having different prevalences. This study aimed to determine the association of rs9939609 polymorphism with obesity in the Balinese. 574 archived specimens collected previously from four villages (Legian, Penglipuran, Nusa Ceningan, and Pedawa) in Bali were employed. Classification for obesity based on body mass index (BMI) and central obesity based on waist circumference (WC) adhered to WHO classification for Asian: BMI ≥ 25 kg/m2 for both male and female, WC ≥ 90 cm for male and WC ≥ 80 cm for female. The rs9939609 genotypes were determined using PCR amplification refractory mutation system (ARMS) technique. Association analysis of rs9939609 SNP with obesity and central obesity were performed by ordinal logistic regression test with adjustments for age and gender. The minor allele frequency of rs9939609 SNP was 0.43, and the genotype distribution did not depart from Hardy-Weinberg equilibrium (p = 0.396). The rs9939609 SNP was associated with obesity in dominant model (coefficient = 0.44, p = 0.040). There was no association of rs9939609 SNP with central obesity. The rs9939609 of the FTO gene is associated with obesity in the Balinese population.
