Abstract
Penelitian ini bertujuan untuk mengidentifikasi faktor-faktor penyebab ketidaksesuaian antara stok fisik di gudang dengan stok yang tercatat pada sistem persediaan PT XYZ. Penelitian ini menggunakan metode deskriptif dengan pendekatan kualitatif, melalui wawancara berbasis kuesioner kepada enam narasumber kunci, serta dianalisis menggunakan metode 5W (What, Who, When, Where, dan Why) dan diagram fishbone untuk mengidentifikasi faktor penyebab secara sistematis. Hasil penelitian menunjukkan bahwa ketidaksesuaian stok dipengaruhi oleh lima faktor utama, yaitu manusia, metode kerja, sistem, material, dan lingkungan kerja. Ketidaktepatan dalam pembaruan data stok, human error pada proses pencatatan, keterbatasan sistem persediaan, serta tingginya dinamika transaksi barang fast-moving menjadi penyebab dominan terjadinya ketidaksesuaian stok. Kondisi tersebut berdampak pada meningkatnya pembatalan pesanan, terganggunya alur distribusi, serta menurunnya kinerja operasional perusahaan secara keseluruhan. Kata kunci: Ketidaksesuaian Stok, SOP Pengendalian Akurasi Stok, Gudang, Fishbone Diagram, 5W Analysis. ***** This study aims to identify the factors causing discrepancies between physical stock in the warehouse and inventory records in the inventory system of PT XYZ, considering that inventory data accuracy plays a crucial role in supporting operational effectiveness and managerial decision-making. Recurrent inventory discrepancies may lead to operational losses and a decline in customer service performance. This research employs a descriptive method with a qualitative approach, using questionnaire-based interviews conducted with six key informants directly involved in warehouse inventory management processes. The collected data were analyzed using the 5W (What, Who, When, Where, Why) method to explore root causes in depth, and further supported by a fishbone diagram to systematically classify and visualize the contributing factors. The results indicate that inventory discrepancies are influenced by five main factors: human resources, work methods, systems, materials, and the work environment. Inaccurate stock updates, human error during recording activities, limitations of the inventory system, and the high frequency of fast-moving transactions were identified as the dominant causes of inventory mismatches. These issues have led to increased order cancellations, disruptions in distribution processes, and a decline in overall operational performance.
