Abstract
Pneumonia adalah kondisi peradangan akut pada jaringan paru yang masih menjadi penyebab utama kematian pada balita dan lanjut usia, dengan pola penularan yang berbeda antar kelompok umur akibat perbedaan daya tahan tubuh, transmisi, serta risiko rawat inap. Penelitian ini bertujuan mengembangkan model matematika penyebaran penyakit pneumonia dengan pendekatan age-structured SVEIHR (Susceptible, Vaccinated, Exposed, Infected, Hospitalized, Recovered) yang membagi populasi menjadi kelompok balita dan lansia, serta mempertimbangkan pengaruh strategi kontrol berupa vaksinasi dan terapi. Penelitian ini dilakukan dengan menentukan titik kesetimbangan bebas penyakit dan titik kesetimbangan endemik, menghitung bilangan reproduksi dasar (R0) menggunakan metode matriks generasi selanjutnya, menganalisis kestabilan kedua titik kesetimbangan, merumuskan strategi kontrol optimal menggunakan Prinsip Minimum Pontryagin, serta melakukan simulasi numerik dan analisis sensitivitas. Hasil penelitian menunjukkan bahwa titik kesetimbangan bebas penyakit bersifat stabil asimtotik lokal ketika R0<1. Tanpa penerapan kontrol, diperoleh R0=2,165887>1, yang menunjukkan bahwa penyakit bersifat endemik dan tetap bertahan dalam populasi. Sebagai skenario batas, ketika kontrol vaksinasi diterapkan secara maksimal dan konstan (u1=1), R0 menurun signifikan menjadi 0,038360, yang menunjukkan bahwa penyakit berpotensi menghilang dari populasi. Selain itu, analisis sensitivitas menunjukkan bahwa sebelum kontrol diterapkan, laju individu terinfeksi yang dirawat di rumah sakit merupakan parameter yang paling berpengaruh dalam menekan penyebaran penyakit, sedangkan setelah kontrol diterapkan, intensitas vaksinasi dan laju kehilangan kekebalan vaksin menjadi parameter yang paling dominan. Simulasi kontrol optimal menggunakan metode forward backward sweep berdasarkan Prinsip Minimum Pontryagin menunjukkan bahwa kontrol vaksinasi bekerja dengan menekan laju penularan sejak kompartemen rentan, sehingga populasi terinfeksi pada kedua kelompok umur tidak pernah mengalami gelombang epidemi, sedangkan kontrol terapi bekerja pada tingkat hospitalisasi dengan mempercepat pemulihan tanpa memengaruhi laju penularan baru. Penerapan kombinasi kedua kontrol menghasilkan penekanan penyebaran penyakit yang paling optimal pada kedua kelompok umur, dengan beban penyakit yang secara konsisten lebih tinggi pada kelompok lansia dibandingkan balita. ***** Pneumonia is an acute inflammatory condition of the lung tissue that remains one of the leading causes of death among toddlers and the elderly, with transmission patterns differing between age groups due to differences in immunity, transmission rate, and risk of hospitalization. This study aims to develop a mathematical model of pneumonia transmission using an age-structured SVEIHR (Susceptible, Vaccinated, Exposed, Infected, Hospitalized, Recovered) approach that divides the population into toddler and elderly groups, while also considering the effect of control strategies in the form of vaccination and therapy. This research was carried out by determining the disease-free and endemic equilibrium points, computing the basic reproduction number (R0) using the Next Generation Matrix method, analyzing the stability of both equilibrium points, formulating an optimal control strategy using Pontryagin’s Minimum Principle, and performing numerical simulation and sensitivity analysis. The results show that the disease-free equilibrium point is locally asymptotically stable when R0<1. Without any control intervention, R0=2,165887>1 was obtained, indicating that the disease is endemic and persists within the population. As a boundary scenario, when vaccination control is applied at its maximum and held constant (u1=1), R0 decreases significantly to 0,038360, indicating that the disease has the potential to be eliminated from the population. Furthermore, sensitivity analysis shows that prior to control implementation, the hospitalization rate of infected individuals is the most influential parameter in suppressing disease spread, whereas after control implementation, vaccination intensity and the vaccine waning rate become the most dominant parameters. Optimal control simulation using the forward-backward sweep method based on Pontryagin’s Minimum Principle shows that vaccination control works by suppressing the transmission rate from the susceptible compartment, so that the infected population in both age groups never experiences a subsequent epidemic wave, while therapy control operates at the hospitalization level by accelerating recovery without affecting the rate of new transmission. The combined application of both controls yields the most optimal suppression of disease spread in both age groups, with the disease burden consistently higher in the elderly group compared to the toddler group.
